With the New Year comes new status for The Free Clinic; it was recognized by the National Committee for Quality Assurance as a Level Three Patient-Centered Medical Home (PCMH), the highest possible level of accreditation.
This designation is the culmination of more than a year’s worth of work as a team of staff members implemented fresh approaches to serving patients incorporating the PCMH concepts of access to care, communication, care coordination and whole person care. “In short, this means that we have established practices designed to make certain that patients will work together with their provider and medical care team to ensure that the patient is receiving the highest quality care and ongoing care coordination that will result in improved health outcomes,” said Farah Munir, the clinic’s Chief Wellness Officer. “We have always been committed to delivering quality care. The PCMH model of care emphasizes putting the patient’s perceptions and needs at the heart of the process so that he or she understands the care plan and is fully participating in carrying it out; it’s a subtle but significant shift in focus,” she said.
“This recognition reaffirms that this health center is living up to its mission to deliver the best possible care to the individuals and families we serve,” said Executive Director Danny Williams. “I am proud of the staff whose diligence earned this recognition and I am gratified by the vote of confidence this signals to our patients and future patients that they can trust that their health and well-being is our top priority.”
