Circle Health Services offers the following resources as wrap-around services to our patients.

Ohio Benefit Bank

A program of the Ohio Association of Food Banks, the Ohio Benefit Bank allows people to sign up for benefits that can help them stretch their household budgets. It offers assistance with food and heating expenses, child care, and access to health care, among others. For more information, visit www.ohiobenefits.org.

Mental Health Resources crisis caller

Having a difficult time? We can help.  In case of emergency, please call 911 or Mobile Crisis at (216) 623-6888; they are available 24 hours a day, 7 days a week.

For counseling or psychiatry services at Circle Health Services, call (216) 721-4010 ext. 1626.  For Substance Abuse Treatment, call (216) 721-4010 ext. 1709.  Leave a message with your name, phone number, and the reason for your call in order to schedule an intake appointment.

CHAP

Cuyahoga Health Access Partnership (CHAP) is a collaboration between local charitable primary care and specialty care providers that gives patients access to the specialty care they may need. If a Circle Health Services provider determines that a specialty care referral is necessary, patients can enroll in the CHAP program, eliminating the need to go through the financial rating process at multiple institutions.  For more information, visit cuyahogahealthaccess.org.

Outreach and Enrollment

Circle Health Services is able to help patients and members of the community enroll in Medicaid or affordable health insurance plans through the Affordable Care Act’s health insurance marketplace. Social Service Program

Circle Health Services is committed to providing high quality care to our clients, and we understand that you have other needs as well.   All Circle Health Services patients have the opportunity to meet our Financial Counselor as well as members of our Social Service team.  Through the assistance of a short questionnaire, the Social Service Program helps connect you with departmental referrals within Circle Health Services as well as resources in the Cleveland community.   From basic needs to workforce development, we have the resources to connect you, and by doing so, we at Circle Health Services hope to support you in reaching your healthy living goals.  Registration appointments and walk-in hours are available Monday – Friday between 9:30am – 2:30pm.
